Some things are better left unsaid. As im sure you know, 18 year old guys spend nearly as much time primping as 18 year old girls, and it seems to me that if you've noticed it he probaly has as well. However if you feel that you should talk to him about it try to be sensitive (if he has noticed he may be embarassed about it) and maybe offer any advice and support you can. Be honest but take his feelings into consideration.
That is a problem. Steroid use can cause this. I'm sure your friend already noticed this, so you really don't need to tell him. But, if you know he is using drugs you should help him the best you can : )

Don't you think he knows?! Or has he never looked in a mirror? Why do you need to tell him? It is unlikely he can do anything about it, so how would it benefit him if you pointed it out? It wouldn't. It would just let him know that it's bad enough for other people to notice and comment on... nice.

if it looks absolutely terrible, tell him nicely - however he may be aware already and like the way it looks - tread carefully on this subject, as people can be very sensitive to these things.
